Bentlybro 26490e32d8 fix(schema): add composite index for LlmModelMigration active queries

Adds @@index([sourceModelSlug, isReverted]) to match the migration SQL.
This prevents Prisma migration conflicts and optimizes queries for
non-reverted migrations by source model slug.
