53aa4e6191 Refactor rollup config of format-title 
This includes using the official rollup plugins where possible, using
dedicated builds for node and browsers and minifying production builds.

ba4605752b Only select columns from BASE TABLEs to exclude views 
Postgres views cannot have primary keys so presence of any views prevented the use of application. Fixes #3229.
