All mentions and links to the Meteor Toolbox VS Code extension have been removed from documentation and tutorials. Based on https://forums.meteor.com/t/vs-code-extension/64106/2?u=grubba, and since the extension repository appears to be unmaintained, it seems best to remove it from the docs.
