update vault to 1.33
create a new user
try to set up duo as 2fa

Expected Result

Seeing popup where the credentials are prefilled with global settings and just click activate

Actual Result

Seeing empty fields
Just in the api host name there stands if you dont touch you are using global settings

Even if you try to fill in the credentials from duo admin. There comes an unexpected error message
