Zamil Majdy 7d96df5f35 fix: prevent duplicate assistant message for long-running tools

Remove the tool_call from accumulated_tool_calls when we detect an
operation_started response, since the assistant message was already
saved in _yield_tool_call. This prevents double-saving.
